Referrals come from many directions at once: teachers, counselors, community partners, and families themselves. This package gives your team one consistent way to receive them, plus a guided intake process that can bring a whole family into Salesforce at once. The result is data that looks the same whether it's entered at the district office, a school site, or a partner organization.

Standard Components

• One external-facing referral form that school staff, partners, or families can use to refer a student or family, either for the first time or from one program to another.

• A referral processing workflow with standard statuses, an email alert to the assigned staff member or team when a new referral arrives, and a “New Referrals” view for triage.

• One internal family intake form that captures parents or guardians and every student in the household in a single process. Standard household fields can be toggled on or off, including household size, number of children under 18, household annual income, and housing status.

• The family form also collects standard biographic and demographic data on each individual in the household, with dynamic questions depending on the household member’s age. Standard fields can be toggled on or off, including race, ethnicity, gender, primary language, address, school, and grade.

• Deduplication automations that check for existing students, guardians, and households before creating new records and update the existing record when a match is found.

• The intake form can be used to periodically refresh family and student information, and our page layout includes an indicator of the last time the family completed the intake.
